It's a Canadian thing. The most popular coffee/doughnut shop is Tim Hortons; their version of "doughnut holes" are called Timbits. The inspiration for the joke is that a friend of mine is currently in hospital, and yesterday had a craving for Timbits from the coffee shop near the hospital entrance.
